[ Summary ] There is no difference in the prognosis after endoscopic therapy for early gastric cancer between patients over the age of 75 and those who are younger. Elderly patients' conditions may easily deteriorate. Therefore, it is thought that patients older than 75 should be treated with endoscopic therapy which procedures less stress and requires shorter hospital stays. When we perform endoscopic therapy we should avoid the accidental complication because the complication comes easily to be severe in elderly patients. It may be difficult provide continued surveillance of elderly patients after endocsopic therapy, but the continuous follow-up after endoscopic therapy for early gastric cancer should be done in elderly patients because metachronous early gastric cancer is more popular in elderly patients than those who are younger.
