[ Summary ] This paper reviews several topics related to problems with biopsies for Barrett's esophagus. The longitudinal (palisade) vessels of the esophagogastric junction provide a guide in the diagnosis of Barrett's esophagus. The presence of ducts of esophageal glands deemed proper in biopsy specimens can assist in the histological diagnosis of Barrett's esophagus. It is important to recognize the dysplasia associated with Barrett's esophagus in the surveillance of incidental carcinoma. Since esophageal cancer can occur at various sites within Barrett's esophagus locations, it is necessary to conduct a detailed inspection of all Barrett's esophagus areas, and to clearly inform pathologists of all relevant information (purpose, methods, site, history and findings).
