[ Summary ] Practical training methods for endoscopic submucosal dissection (ESD) were introduced at Saku Central Hospital. The hospital is a high volume center for EGD (over 23,000 cases per year). The trainees can develop the endoscopic skills necessary for ESD via screening EGD. The diagnostic abilities required for ESD are acquired through discussions at case conferences held three times a week. Knowledge and necessary skills for ESD are enriched by working as assistants during the procedure, followed by experimental practice, using a resected porcine stomach. Then, clinical training in ESD is carried out with the step-up system, under supervision of a trainer. The 15-minute-5 minute rule works very well for the recognition of how to overcome difficulties during ESD and improvement of technical ESD issues.
