[ Summary ] Peritoneal metastasis is one of the most common types of recurrence after radical surgery for carcinoma of the digestive organs, and the cytology of peritoneal lavage fluids at the time of surgery (CY) is a very useful predictor for peritoneum recurrence. CY positive is associated with poor prognose in gastric cancer. Subclassfication, according to cluster formation can predict more accuratly the outcome of CY positive cases. Thus, we perform standard radical surgery for CY 1 patients with negative cluster formation, and choose limited surgery in patients with positive cluster formation. The effects of intraperitoneal hyperthermic infusion therapy in CY positive cases remains unclear. Although CY may be a useful predictor for peritoneal recurrence after curative surgery in patients with colon, pancreatic and biliary tract cancer, further investigation is required to clarify the evidence.
