[ Summary ] Liver resection is the only treatment that offers a cure for patients with liver metastasis from gastrointestinal malignancies. The prognosis after hepatectomy depends on the primary site. Liver resection of colorectal metastases is widely accepted. The presence of bilobar metastases is not necessarily a contraindication for liver resection, 5-year survivors are not uncommon in patients with multiple metastases. In a collected series of resected liver metastases from gastric carcinoma, long-term survivors were uncommon. However, in a series of highly selected cases, hepatectomy contributed to the 5-year survival rate. In experienced hands, liver resection is safe. In the future, surgical therapy will play a more important role in liver metastasis.
