[ Summary ] Usefulness of various transparent cylinders and their clinical application for colonic disease was discussed.
Colonoscopic therapy, using transparent cylinders is divided for two major categories. One is resection of colonic tumors and the other is endoscopic hemostasis.
Resection techniques for colonic tumors include EMR using oblique transparent cylinders with local injection of Glyceol for superficial epithelial colonic tumors and endoscopic aspiration mucosectomy (EAM) for small carcinoid tumors in the lower rectum. Long lasting high mucosal protrusions made by local injection of Glyceol and easy approaches to the tumor using oblique transparent cylinders contribute to safe, reliable and short procedures. Aspiration methods contribute to secure vertical resection margins. Each procedure is a novel technique.
The merits of endoscopic hemostasis using transparent cylinders are easy approaches to the bleeding lesion and protection of various devices in the cylinders. An easy approach to the lesion provides an adequate water jet and targeting of the lesion for hemostatic devices. Protection of hemostatic devices contributes to reduction of both needless injury to the lesion and prevention of unexpected closure of the hemoclip hy contact with the lesion.
