[ Summary ] At present, early colorectal cancers with submucosal invasion up to 1,000 μm are considered to be an indication for endoscopic resection. Considering that the shape of the submucosa is modified by the shape of lesions and degree of cancer invasion, EUS diagnosis of submucosal invasion depth should be made based on the patterns from EUS images. However, estimation of invasion depth is possible by measuring the depth of the hypoechoic area for lesions with shallow submucosal invasion. Consequently, EUS can easily adapt to changes in criteria in contrast to other diagnostic modalities. Furthermore, by analyzing the features of the hypoechoic area in the third layer, an estimation of histology may be possible. In the future, improving diagnostic accuracy should be attempted by integrating information from other modalities.
