| [ Summary ] | Triple therapy (proton pump inhibitor + amoxicillin + clarithromycin) is the first-line treatment for Helicobacter pylori (H. pylori) infection in Japan. However, the effectiveness of this therapy is decreasing due to an increase in refractory H. pylori. After the failure of the first-line therapy, retreatment with proton pump inhibitor+ amoxicillin + metronidazole has proven to be effective, and it was recommended as a second-line regimen by the Japanese Society for Helicobacter Research in 2003. The best approach for survived H. pylori after these therapies is antimicrobial susceptibility testing. Treatment should be thought of as a ‘treatment package’ which consists of first-, second-and post second-line eradication therapies as a whole, such as those seen in the Maastricht 2-2000 Consensus Report. | |
