[ Summary ] Research on the establishment of guidelines for gastric ulcer treatment has recently been performed by the Evidence Based Medicine Working Group in Japan. Based on clinical evidence regarding the treatment of gastric ulcers in patients on long term non-steroidal anti-inflammatory drug (NSAID) therapy, the following statement has been recommended: (1) conventional treatment for gastric ulcers should be performed, if NSAID can be withdrawn; (2) either a proton pump inhibitor (PPI) or prostaglandin (PG) should be prescribed unless NSAID can be withdrawn; (3) high doses of H2 receptor antagonist, PG and PPI are effective for the prevention of recurrence of healed gastric ulcers in patients on NSAID; (4) there is no clinical evidence to indicate that the eradication of Helicobacter pylori is beneficial for ulcer healing or prevention of recurrence of healed ulcers in patients on NSAID.
