[ Summary ] Endoscopic mucosal resection (EMR) is a well established treatment for early gastric cancer (EGC) in Japan. Endoscopic submucosal dissection (ESD) is a new EMR technique and is aimed at obtaining one piece resection even in large lesions. At our hospital we performed ESD using an IT knife and in the last three years we have treated 1,033 EGC patients. In this study we reviewed the technical feasibility and complication rates associated with ESD, using an IT knife. Additionally, we assessed the use of an electrosurgical unit (VIO 300D; ERBE). This new equipment may reduce not only the risk of blood loss during ESD but also reduce procedure time. ESD is a new technique and requires significant skill, training and equipment set up to deal with related complications.
