[ Summary ] Conventional endoscopic mucosal resection (EMR) techniques make it possible to resect only small specimens. Therefor, piecemeal resection has been performed for large lesions.
Now, we have developed a new endoluminal surgery method-endoscopic submucosal dissection (ESD).
We cut the mucosa around the lesion with a hook knife, and dissect submucosal fibers, also. Then, a large lesion can be resected in one piece. We have resected seventy six cases superficial cancer with this new technique. The one piece resection rate was 89.50% (68/76). The major complication was mediastinal emphysema 5.30% (4/76), and there were no perforations.
A more precise and larger one piece resection could be performed with ESD. Therefor, ESD with a hook knife is useful for treatment of superficial esophageal cancer.
