[ Summary ] With the advent of multi-detector row CT (MD-CT), more distinct axial images, multiplanar reconstruction images (MPR), curved MPR or 3D images of the bile duct can be obtained. Compared to conventional CT, MD-CT can better evaluate tumor location, or involvement of the hepatic artery and the portal vein, but diagnosis of proximal or distal tumor spread along the bile duct is still difficult to make. MPR or curved MPR can delineate the bile duct along its long axis and may be useful in evaluating the relationship between a tumor and its surrounding structures. 3D images of the bile duct can be reconstructed with surface rendering methods after percutaneous transhepatic cholangiography (PTC) or drip infusion cholangiography (DIC). Better 3D images and virtual endoscopy of the bile duct depend on an appropriate thresh old level, and the whole image of the bile duct can be visualized, but characterization of lesions is difficult to make. Images of the bile duct reconstructed with MD-CT can be informative.
