[ Summary ] Liver transplantation is widely accepted as the only therapeutic modality for end stage primary biliary cirrhosis. In Western countries, the prognosis of the disease, using mathematical models, including Mayorisk scores after liver transplantation, have been fully examined. The long term survival rate after transplantation in large series is around 80% and in Japan the series after living donor liver transplantation is a little poorer, 70%. The problems with outcome of living donor liver transplantation may be overcome by recent surgical advances or knowledge regarding the appropriate timing for transplantation among hepatologists.
