[ Summary ] In order to safely and satisfactorily diagnose and treat hilar cholangiocarcinoma, it is important to understand the detailed anatomy of both the portal and biliary systems, and their interrelationships. Recent advances in helical computed tomography(CT) have made it possible to easily reconstruct accurate and realistic three-dimensional(3D) images from arbitrary angles, which facilitate comprehension of the complex anatomical structures of the hepatichilum. From helical CT during both arterial portography and cholangiography, 3D porto-cholangiograms were reconstructed to verify the relationship between the portal and biliary systems. 3D porto-cholangiography is very useful for assessment of hilar cholangiocarcinoma, and we consider recognition of the relationship between the portal and biliary systems to be clinically important not only for preoperative evaluation of patients with hilar cholangiocarcinoma but also to reduce organ liver transplantation.
