[ Summary ] We review the efficacy of PPIs in the long-term treatment of acid-related disease (reflux esophagitis, peptic ulcers, and Zollinger-Ellison syndrome). Long-term treatment with PPIs is superior to H2RA or placebos in terms of healing and decreasing relapse rates. For reflux esophagitis, half-dose treatments are also effective and safe. We review Japanese and other country's papers, which have reported on half-dose therapy for GERD. For peptic ulcers, this therapy is also sometimes necessary for maintenance. In Japan, PPIs are not available for use in maintenance therapy for peptic ulcers. Patients with Zollinger-Ellison syndrome need high-doses of PPIs. High-dose and prolonged treatment with PPIs are reported to be safe and effective in the control of acid secretion.
