[ Summary ] Anomalous pancreaticobiliary ductal union (APBD) is a high risk factor for biliary malignancies. Many hypotheses have been proposed to explain the pathogenesis of biliary carcinoma in patients with APBD. Accumulating evidence suggest the significance and prevalence of epithelial hyperplasia in the pathogenesis of gallbladder carcinoma in APBD patients as a possible precursor lesion related to gallbladder carcinoma. Biliary carcinoma developing after excision of choledochal cysts is discussed. In addition to pancreatic juice, reflux of intestinal contents into the biliary tree causes changes in the biliary epithelium that have the potential to lead to malignant transformations.
