[ Summary ] The severity of acute severe pancreatitis should be assessed as early as possible to improve prognoses, especially mortality rates. We have evaluated some scoring scales for prediction of disease prognosis, including the total prognostic scores (TPS) proposed by a research group of the Ministry of Health and Welfare of Japan, Ranson scores, APACHE II scores and scores predicting the most severe acute pancreatitis. The borderline score in TPS is 7-8, which indicates the critical point of death. The power of predicting the prognosis of the disease with both TPS and APACHE II scores is superior to that with Ranson scores. Scores predicting the most severe forms of the disease may serve as a simple indicator for predicting the prognosis (mortality) and complications, including infections and organ dysfunctions. A simpler but more reliable scoring scale should be developed as soon as possible.
