[ Summary ] Hepatic arterial infusion chemotherapy (HAI) is the treatment of choice for unresectable hepatic metastasis from colorectal cancer. A rationale for use of HAI may be developed from both anatomical and pharmacological aspects. Weekly high-dose HAI of 5-fluorouracil is commonly used in Japan. Several randomized controlled trials and two meta-analyses showed that HAI displayed higher response rates without increased survival benefits, when compared with HAI and systemic chemotherapy. To truly define the role of HAI, predictive and prognostic factors associated with HAI should be studied.
