[ Summary ] | Endoscopic ultrasonography (EUS) has been developed with the aim of detecting small pancreatic carcinomas. In addition, hased on the advances in technology, small diameter ultrasonic probes, which can pass through the working channel of ordinary endoscope, have heen developed. By using this instrument, intrabilio-pancreatic ductal ultrasonography (IDUS) can be performed, inserting the probe into the biliopancreatic duct, under duodenoscopic guidance. Indication for use of IDUS for pancreatic lesions is limited because of the difficulty of inserting the probe into the pancreatic duct and the possibility of post examination pancreatitis. However, by using this method, small tumorous lesions in the cystic lesions of mucinous cystic tumors can be detected and the staging of pancreatic ductal carcinoma is also possible from the precise images obtained by IDUS study. Furthermore, three dimentional images can be constructed by special models of ultrasonic probe. IDUS is one hopeful method for diagnosing the small lesions of the pancreas, though there are some unsolved problems and hazzards when inserting the probe into the pancreatic duct. |
