[ Summary ] 2cm of anal clearance margin instead of 5cm for lower rectal cancer increased the indications for sphincter-saving operations. The intramural spread of cancer beyond 2cm is related to tumor size, circumference, appearance, and differentiation.
After very low anterior resection, the most popular sphincter-saving operation, patients suffered from defecatory dysfunctions, such as bowel frequency, in complete evacuation, or soiling, which reduce the quality of life. Colonic J-pouch operations are effective in reducing these functional problems. The best operational procedures, indications, and physiological background for pouch operations is under investigation.
