[ Summary ] The indication of tumor resection by using the electric endoscope is mucosive or submucosive invasive cancer. For diagnosis, observation with dye reagents and/or observation about superficial structures (pit pattern) must be done. Tumors showing sessile growth are resected using the EMR technique, while tumors displaying polypoid growth are resected, after ligation, using a clip or detention snare, for the purpose of prevent complications. After resection, if the specimen is diagnosed as submucosive invasive cancer having the potential for lymph node metastasis, additional resection with lymph node dissection would be recommended. We consider the tumor having 4 factors among 5 factors (middle or deep submucosive invasion, moderate or poor differentiation, poorly demarcated growth, budding, and vessel permeation) as high-risk tumors for lymph node metastasis.
