[ Summary ] Laparoscopic procedures used for esophagectomy are expected to improve temporarily debilitating postoperative pulmonary functions. However, a simple and secure operative procedure has not been established. Mini-laparotomy or hand-assisted procedure contribute to the feasibility of this procedure and to the shortening of operation time. The authors do not adopt the use of mini-laparotomy, but insert a spacer-balloon into the omental sac to simplify gastric mobilization. Technical innovations are mandatory for reconstructing the gastric tube and for lifting up to the neck.
