[ Summary ] The diagnosis of irritable bowel syndrome (IBS) is based on symptom-based criteria, excluding other conditions with similar clinical presentations. Abnormal motility of the digestive tract, enhanced visceral sensitivity, and psychosocial factors are related to the pathophysiology of IBS. The treatment strategy is based on the severity of the symptoms and the degree of the psychological state from a psychosomatic viewpoint. Patients with mild illness require life style modification and pharmacological treatment directed toward altering gut physiology. Patients with moderate and severe symptoms require psychotropic medications and psychologic treatments, The goal of treatment is not to cure but to develop self-control of symptoms and the acquirement of good social skills.
