[ Summary ] Unfused pancreatic duct system is defined as a congenital anomalies which shows non-fusion or incomplete-fusion between ventral and dorsal pancreatic duct. Pancreatic juice secreted from dorsal pancreas is drained via relatively narrow dorsal pancreatic duct and accessory papilla. In addition to these anomalies, stimulation to exocrine secretion or inflammatory narrowing of the accessory papilla produce clinical symptoms such as acute pancreatitis or abdominal attack. Unfused pancreatic duct system is the most frequent disease in congenital anomalies of the pancreas and the proportion of unfused pancreatic duct system in acute pancreatitis is about 3%. A detailed analysis of ERCP is necessary for the diagnosis and surgical accessory duct sphincteroplasty is useful for the patients with pancreatitis due to insufficient drainage of pancreatic juice.
