[ Summary ] Choledochal cyst is a congenital disease and common in females (female:male, 4:1) with higher prevalence in Japan and Orientals. This disease is frequently associated with anoma1ous junction of the pancreatic duct and common bile duct which causes the arising of gallbladder and common bile duct cancer. The principle of surgical procedure is cyst excision and reconstruction of biliary tract. Todani III (Alonso-Lej III) type is very rare ma1formation and the clinical features and the conservative treatment for this type were discussed.
