[ Summary ] We investigated the efficacy of a traditional herbal medicine, high-dose-treatment (HD-T) with Oren-gedoku-to (OGT) (15.0g-5.0g/day), in terms of in vitro activity against Helicobacter pylori (H.pylori), and observed the long-term course of H.pylori-reinfection after eradication. The eradication rate achieved with OGT was highest (19/20 : 95.0%) in patients with intractable gastric ulcer, followed by (37/ 50 : 78.0%) younger and middle aged patients with gastric ulcer recurrence (GUR) and was also excellent (47/50 : 94.0%) in elderly patients with GUR at 2years after OGT-HD-T. Significant eradication of H.pylori was observed at 2years after OGT-HD-T in young and middle age-patients with GUR and at 2weeks after HD-OGT-T in elderly patients. The reason for this therapeutic difference between the two age groups is unknown. Patients showing effective eradication of H.pylori by OGT are obese, hypertensive and have phsycological profiles contributing to GUR. Clinical and basic studies aimed at detecting new kampo-combination therapies are now underway.
