[ Summary ] For the early diagnosis of carcinoma of the papilla of Vater, patients without jaundice should receive a check-up using ultrasonographic for screening or blood examination. Moreover, in routine upper digestive tract endoscopy, careful observation in the region of the duodenal papilla appears to be very important. When abnormalities in such cases are detected, further examination by endoscopic retrograde cholangio-pansphincteromy(EST) is prerequisite for an accurate biopsy of the leslon. Clinicopathologically, differential diagnosis between adenoma and carcinoma of the papilla of Vater is extremely difficult. In such cases, repeated biopsies or endoscopic papillectomy may be necessary to make and confirm the final diagnosis and to decide upon any additional treatments.
