[ Summary ] We performed laparoscopic adhesiolysis on 19 cases of adhesive ileus using mainly a minilaparoscope. We successfully operated on 14 cases but small intestinal injury necessitated a minilaparotomy in 5 cases. The cause of small intestinal injuries was inappropriate use of intestinal holding forceps in all 5 cases. Postoperative courses were satisfactory in all 19 cases and there have been no recurrences of adhesive ileus for a period of 2 months to 5 years. The advantage of this procedure is that it does not increase intestinal adhesion. Though laparoscopic adhesiolysis is a simple and safe procedure, minilaparotomy should be selected if there is a possibility of procedure-related intestinal injury.
