[ Summary ] Minimization and simplicity are the basic princlples of prescription for the elderly. The H2-blockers and PPIs are very effective even for elderly subjects whose gastrlc acid secretion is often low. Since the H2-blockers are excreted via the kidney, doses should be reduced by half. The PPIs are useful for reflux esophagitis. However guidance in daily living is essential. When antacids containing anti-cholinergics are taken for a long time, increments in intra-ocular pressure and tachycardia may occur. NUD (non acid dyspepsia) in the elderly is usually hypochondriacal in nature, but antipsychotic drugs should be prescribed with caution. Repeated and sincere hearing of complaints is essential. When plykinetic drugs are given, extrapyramidal symptoms must be considered as potential side-effects. Constipation is common in the elderly and laxatives may be prescribed along with encouragement to take large volumes of water and consume a fiber-rich diet.
