[ Summary ] Many studies have reported an association between hyperphosphatemia and mortality, though none have demonstrated that lowering serum phosphate level directly leads to improved survival. Indeed, it may even lead to increased mortality. These facts suggest that nutritional status of patients should be considered primarily, before hyperphosphatemia is treated. Avoiding excessive phosphate restriction, removal of phosphate by dialysis, and treatment with appropriate phosphate binders would lead to better survival in dialysis patients. It is also crucial to consider the patient's background, parathyroid function, and bone metabolism when managing serum phosphate, calcium, and parathyroid hormone levels.
