【 要旨 】 シナカルセトは，二次性副甲状腺機能亢進症を合併した維持血液透析患者の血清リン値を低下させるが，保存期腎不全患者ではリン利尿作用をもつPTHの抑制を介して血清リン値が上昇する．そこで，新規にシナカルセトを投与したPD患者8名を対象に，投与1カ月後の血清Ca・リン・iPTH値の変化と1日尿量との関係を後方視的に検討した．投与1カ月後，iPTH値は中央値374（241～886）pg/mlから中央値183（35～622）pg/ml まで低下し，血清Caは全例で有意な低下が認められた（それぞれp＜0.05）．一方で，血清リン値の変化は残腎機能に依存しており，尿量が750ml/日未満のPD患者4名では有意に低下したが，尿量が750ml/日以上の4名では増加した．以上より，残腎機能のあるPD患者ではシナカルセトによるリンの低下は期待できず，治療の際に考慮すべきと考えられる．
