【 要旨 】 副甲状腺機能低下症はPTHの分泌低下あるいは作用不全により低Ca血症，高P血症を起こす疾患である．これまで特発性副甲状腺機能低下症とされてきたもののなかにカルシウム感知受容体，TBX1，GCMB，GATA3，TBCE，PTHの遺伝子異常によるものや自己免疫機序によるものが含まれる．偽性副甲状腺機能低下症の病因として，Gs蛋白をコードするGNAS遺伝子の変異あるいはその近傍のメチル化異常が重要であることが明らかとなった．

[ Summary ] Hypoparathyroidism is a disease associated with hypocalcaemia and hyperphosphatasia caused by a decrease in PTH secretion or PTH resistance. Genetic disorders in the calcium-sensing receptors, TBX1, GCMB, GATA3, TBCE and PTH as well as autoimmune disease are included in previously described idiopathic hypoparathyroidism related conditions. Recent evidence indicates that abnormal GNAS gene encoding for the Gs protein and methylation abnormalities in the GNAS locus are important for the molecular pathogenesis of pseudohypoparathyroidism.
