題名 保存期から始まるCKD-MBD管理

【 要旨 】 慢性腎臓病に伴う骨・ミネラル代謝異常（CKD-MBD）は，透析導入前の保存期から連続的に生じる病態として認識する必要がある．近年，CKD-MBDが生命予後に及ぼす影響はすでに保存期の段階から始まっていること，さらにこれらの管理状況が腎予後にも影響を及ぼすことが示され，保存期からの積極的な介入が予後の改善につながる可能性が示唆されている．このような状況のなか，2012年3月に日本透析医学会より「慢性腎臓病に伴う骨・ミネラル代謝異常の診療ガイドライン」が発表された．本ガイドラインにより保存期におけるCKD-MBD診療の質が向上するだけでなく，これを機にわが国からの保存期CKD-MBDにおけるエビデンス発信が促進されることが期待される．

Title Management of CKD-MBD during predialysis period

[ Summary ] Chronic kidney disease-mineral and bone disorder (CKD-MBD) develops early in the course of CKD and should be recognized as a progressive disorder beginning in the predialysis period. Recent epidemiologic data has demonstrated that the association between abnormalities in biochemical parameters of CKD-MBD and mortality and poor renal outcomes can be found in predialysis patients, suggesting that interventions during the predialysis period to control biochemical parameters improve patient-level outcomes. Under these circumstances, the Japanese Society for Dialysis Therapy released "Clinical Practice Guideline for CKD-MBD" in March 2012. It is hoped that this new guideline not only improves management of CKD-MBD in predialysis patients in Japan but also becomes a good chance to produce evidence for predialysis CKD-MBD from Japan.
