Hypercalcemia and hypophosphatemia are common complications after kidney transplantation. Persistent hyperparathyroidism (HPT) and high serum fibroblast growth factor (FGF) 23 are suspected of playing large roles in these conditions. The influence of these hormones differs in relation to the time after transplantation. High FGF23 levels are probably the main regulator of serum phosphate in the short term. Persistent HPT is probably the main factor influencing mineral metabolism after transplantation in the long run. Since this correlates with parathyroid hormone levels before transplantation, it is essential to control pre-transplant HPT. |
