【 要旨 】 わが国の透析患者の死亡原因に占める心血管病変 (CVD) の割合は約39 %と高く，そのリスクは健常人のそれと比較して10 - 20倍高いといわれている．Ca・P代謝異常は従来，主として骨代謝病変 (ROD) との関係で論じられてきたが，近年，多くの観察研究において，血管石灰化をはじめとしたCVD発症に深く関与し，透析患者の生命予後に大きな影響を及ぼしていることが指摘されるようになり，その治療戦略は大きく変わってきている．

[ Summary ] It is said that cardiovascular mortality is greater in hemodialysis patients than in the nonuremic persons, and that they have 10 to 20 times more risk of cardiovascular disease than healthy individuals. Recent observational studies point out that the Ca / P metabolism abnormality participate in the CVD onset, and have an great influence on the mortality of the dialysis patient. Treatment strategy has changed very much.
