| 【 要旨 】 | 透析患者に対する経皮的エタノール注入療法 (PEIT) を安全に遂行し正しく普及させるために，これまで選択的副甲状腺PEITに関するガイドラインが公表されてきた．今回，3回目の改訂版として「選択的副甲状腺局注療法に関するガイドライン」が発表された．このガイドラインでは，内科的治療に抵抗する症例のなかで，とくに結節性過形成が１腺のみ腫大している症例が有効であることが紹介されている．このガイドラインは，2006年に透析医学会から発表された「透析患者における二次性副甲状腺機能亢進症治療ガイドライン」に準拠しており，そのインターベンションの，とくにPEITに関する実際について，さらに詳しく述べた内容となっている．さらに，近年普及しているビタミンD製剤の副甲状腺に対する直接注入療法についても記述されている．ガイドラインの活用と，時代に即した改訂が望まれる． | |

| [ Summary ] | Guidelines for selective percutaneous ethanol injection therapy (PElT) have been released in order to support its appropriate and safe application. Recently, the 3rd edition of the 2006 guideline for selective direct injection therapy of the parathyroid was published. In this guideline, patients with one nodular hyperplasia are introduced as the most suitabte candidates for PElT. This guideline, based on the 2006 guidelines for the management of secondary hyperparathyroidism, which was released from the Japanese Society for Dialysis Therapy (JSDT), describes more details related to the section of intervention in the JSDT guidelines. In addition, this guideline refers to direct vitamin D injections to the parathyroid, which is a method that has been spreading recently. Application and revisions of this guideline are mandatory depending on the current medical trend. |
