Third generation assays have introduced the precise measurement of bioactive 1-84 PTH to clinical practice. However, it is still difficult to control bone metabolism in the appropriate ranges because bone sensitivity to PTH differs by sex and individual. Low PTH levels do not always mean low bone turnover. On the other hand, bone turnover markers reflect bone sensitivity and often provide worthy remarks on bone metabolism. In chronic kidney disease (CKD) patients, however, a careful interpretation of several bone markers is required. The related metabolism is affected by renal functions. In this context, the interpretation of bone markers in patients with CKD, differences of these markers by gender, and gender specific medications for CKD-MBD (CKD-mineral bone disorders) will be discussed. |
