【 要旨 】 血液透析患者の腎性骨症はより重要な問題になってきているが，その診断・治療方針の決定，効果判定，骨代謝のコントロール目標などに副甲状腺ホルモン（PTH）の測定は必要不可欠である．このホルモンは，血中ではwhole moleculeのPTH以外に，種々の短いフラグメントが存在している．現在では，作用や強さ半減期などが異なるさまざまなPTHが測定可能となった．そのため，測定値の解釈や各測定値の組み合わせ方など，多くの未解決部分が存在している．また新しい鋭敏な測定系が開発されることにより，PTH測定条件の統一化など新たな問題が出現する可能性もある．今後データの蓄積と検討により，診断・治療および発症機序の解明が期待される．

[ Summary ] Parathyroidhormone (PTH) is one of the most critical factors for monitoring the bone metabolism in patients who received hemodialysis. However, not only its instability, recent development of various assays make the interpretation of the results more complicated. It is expected to clarify the diagnosis, therapy and mechanisum of the sick by accumulation of PTH datum.
