【 要旨 】 在宅血液透析（HHD）は生命予後も良く，社会復帰率も高く，果たす役割は大きい．HHDにおいてもっとも重要な点は安全性の確保である．そのためには患者・介助者の教育訓練が重要である．HHD導入教育を開始するかどうかは，医師，看護師，臨床工学技士（CE）などで構成する判定会議で決定され，HHD導入教育後，HHDへの移行については移行会議で決定される．必要な時期に複数の医療者が協議することは必要であると考える．本稿では，教育訓練を含めたHHD導入プログラムについて記述し，HHDにおける自己責任と管理責任の問題についても私見を述べた．

[ Summary ] Home hemodialysis (HHD) is associated with a remarkably good survival prognosis. Patients undergoing HHD show a high rate of return to active roles in society. It is expected that HHD will play a greater role in the near future among patients who wish to perform certain functions in society. The most important concern associated with implementation of HHD is ensuring a high level of safety. Therefore education and training is impor tant for patients undergoing HHD. Meetings should be held at the beginning and the end of HHD training. We have outlined a program for the initiation of HHD, including a method for training patients undergoing HHD that additionally discusses self-responsibility.
