【 要旨 】 糖尿病および慢性腎臓病は,ともに脳血管疾患の独立した危険因子であり,糖尿病透析患者は脳血管疾患のハイリスク集団である.とくに,透析患者は血行力学性脳梗塞を発症しやすく,糖尿病透析患者ではさらにそのリスクが高い.脳血管疾患を予防するには高血糖,高血圧といった動脈硬化危険因子の管理に加え,透析後の起立性低血圧の予防が重要である.心房細動合併透析患者の抗凝固療法の是非については未だ結論が出ておらず,今後の新たなエビデンスの蓄積が望まれる.

[ Summary ] Diabetes mellitus and chronic kidney disease are both independent risk factors for ischemic and hemorrhagic stroke. Therefore, diabetic patients undergoing dialysis are at a significantly higher risk of cerebrovascular diseases. The risk of hemodynamic ischemic stroke in diabetic patients undergoing dialysis is significantly higher than that in non-diabetic patients undergoing dialysis. This heightened risk is attributable to severe orthostatic hypotension and reduced cerebral blood flow and oxygen delivery in the former group. Thus, in addition to treatment for hyperglycemia, hypertension, and anemia, the prevention of orthostatic hypotension immediately after hemodialysis is important to prevent stroke in these patients. The use of anticoagulants in patients with atrial fibrillation undergoing hemodialysis remains controversial, because several studies have shown an increased risk of hemorrhagic events in such patients, and the anti-thrombotic effects of these agents are unclear. Further studies are required to prove the propriety of this therapy in these patients.
