| 【 要旨 】 | 腹膜透析（PD）療法は，在宅医療であることが本来の特徴である．超高齢社会において，透析の主たる対象患者は高齢者である．そして，多くの高齢者は，住み慣れた居宅で天寿を全うすることを望んでいる．死は，高齢者が迎える人生最後の出来事であり，人生を全うするための不可欠な瞬間である．多くの合併症，併存症，フレイル，認知症などを抱える高齢者に適しているPD療法は，高齢者が人生を自然に生きて，自然に人生を終えることを可能にしている． 在宅におけるPD患者の看取りは，それを望む高齢PD患者を中心として，家族，医療・看護の担当者による看取り，すなわち“チーム看取り”といえる． そして，医療者に与えられた医療・看護のなかで，“看取り”こそは，もっとも崇高な行為である．看取りは，死の瞬間ではなく，死までの生を看取ることである． |

| [ Summary ] | Peritoneal dialysis (PD) is primarily a home-based medical therapy. In a super-aging society, an elderly patient is the primary target of dialysis. Several elderly patients wish to spend their terminal phase at home in a familiar environment in which they have spent long years of their life, preparing for death -- the ultimate and inevitable event of life. PD therapy is suitable for elderly patients with multiple complications, comorbidities, frailty, and dementia. PD therapy enables an elderly patient to live life naturally until the end. Effective end-of-life care requires teamwork between the elderly patient, his/her family, and the person in charge of medical and nursing care. Providing holistic end-of-life care is a noble act for medical staff. End-of-life care is to be present during life until death and not only at the moment of death. | |
