【 要旨 】 一般人口死因の第1位である悪性新生物(悪性腫瘍)は,透析人口死因では心不全,感染症に次ぐ3位である.Isekiらは,わが国の透析人口の一般人口に対するがん死亡標準化死亡比(SMR)を2.96倍と報告している.海津らは,わが国の透析人口の一般人口に対するがん罹患標準化罹患比(SIR)を,男性1.07,女性1.41と報告している.さらに海津らは,男性で腎がん,多発性骨髄腫の,女性では子宮がんのSIRが高く,また導入直後にがん罹患SIRは高いことを示した.海外からは慢性腎臓病(CKD)や急性腎障害(AKI)既往の存在ががん罹患リスクになることが報告されている.

[ Summary ] Malignancy is the leading cause of death in the Japanese general population, and the third most common cause of death in the Japanese dialysis population. Iseki et al. reported that the standardized mortality ratio for cancer death in the Japanese dialysis population relative to the general population was 2.96. Kaizu et al. reported that the standardized incidence ratio (SIR) for cancer in the Japanese dialysis population relative to the general population was 1.07 for men and 1.41 for women. They also reported that the SIR for renal cancer and multiple myeloma was high in men and that of uterine cancer was high in women. They further reported that the SIR for cancer is highest soon after starting dialysis. It has also been reported that the presence of chronic kidney disease (CKD) and/or acute renal failure (AKI) are risk factors for cancer morbidity.
