【 要旨 】 | 透析患者において食事管理の良否は生命予後に直結する.過剰なエネルギー摂取や食塩摂取はそれぞれ脂質異常症や体液負荷を惹起し,心血管疾患(CVD)のrisk factorとなる.また,たんぱく質の過剰はリンの過剰摂取の原因となって高リン血症を助長し,慢性腎臓病に伴う骨ミネラル代謝異常(CKD-MBD)のrisk factorとなる.これらの改善には,食品の栄養学的特徴,および透析患者の病態を十分に理解したうえで,患者個々の食習慣や生活状況に合わせたテーラーメイドな栄養指導が必要である.そのためには医療スタッフの諦めない支援と指導の工夫が重要である. |

[ Summary ] | Proper nutritional management is crucial for improving prognoses for hemodialysis (HD) patients. Excessive calorie intake, as well as salt intake are known to be risk factors for CVD. These can induce dyslipidemia and volume overload. Excessive intake of proteins may cause hyperphosphatemia, which is considered to be a risk factor for CKD-MBD. Not only an adequate understanding of nutritional profiles for diets and the pathophysiology of HD patients, but also an understanding of individual patient's dietary habits is needed to provide better nutritional management. Ideal dedicated, supported by medical staffs is the key to improving prognoses for HD patients. |
