【 要旨 】 糖尿病性腎症は末期腎不全の最多の要因の一つである．糖尿病性腎症による末期腎不全では透析患者と比較して，腎移植患者の生命予後，その後のQOLは維持されており，糖尿病性腎症においても移植は推奨されるべき治療の一つである．糖尿病性腎症に対して腎移植を施行した患者の死因の第一位は，冠動脈疾患であり，細菌感染や真菌感染のリスクも高いといわれている．これらのリスク軽減は必要であり，術前評価また術後の評価も重要である．また，本稿では糖尿病の透析患者における血糖管理の概要にも言及する．

[ Summary ] Diabetic nephropathy is a leading cause of end-stage renal disease (ESRD). Since kidney transplantation patients have better survival and quality of life than dialysis patients, patients with diabetes mellitus should also receive kidney transplantation. CVD is the most common cause of death among diabetic patients along with bacterial and fungal infection. Therefore, pre-transplantation evaluation and post-transplantation management of diabetic patients are important.
