【 要旨 】 最近,認知症はフレイルの観点からも検討されている.地域居住高齢者と同様,透析患者でもフレイルは認知機能低下と関連する.運動,とくに有酸素運動は,骨格筋や脳内の脳由来神経栄養因子(brain-derived neurotrophic factor)の産生が亢進し,海馬の顆粒細胞が増殖され,学習能力や記憶力を改善させる可能性がある.とくに,運動と認知タスクの両者を同時に行う二重課題トレーニングは,健康高齢者の認知機能を改善することが報告されている.透析患者では,健常人と比較し,二重課題(例:歩行しながら会話する)をこなす能力が低下している.しかし,二重課題トレーニングの効果については検討されていないため,透析患者の認知症の発症・進行を予防する運動プログラムの開発が求められる.

Title Exercise training for dialysis patients with cognitive impairment

[ Summary ] Cognitive impairment is being studied from the viewpoint of patient frailty. As in community-dwelling older adults, frailty is associated with cognitive impairment in dialysis patients. Exercise, especially aerobic exercise, increases the production of brain-derived neurotrophic factors in the muscles and brain, which leads to enhanced neurogenesis in granular brain cells, particularly in the hippocampus, and to improved learning and memory ability. Dual-task training combined with exercise and cognitive tasks is reported to improve cognitive functions in healthy elderly patients. In dialysis patients, walking balance is impaired during dual task performance (e.g. walking and talking) to a greater extent than in age-matched healthy controls subjects. However, there have been no studies to examine the effects of dual-task training on cognitive function. As a result the development of new exercise programs to prevent the onset and/or progression of dementia are needed for dialysis patients.
