【 要旨 】 血液透析患者の生命予後に重要な影響を及ぼすリン(P)代謝は,透析前血清P濃度のみで評価されてきた.血液透析でのP除去量を新たな指標と考え,簡便かつ正確にこれを推定する式を考案した.長期的なP 出納バランスは保たれており,腸管からの吸収量と透析での除去量はほぼ等しい.すなわち,除去量の推定は吸収量の推定を意味する.これにより,P吸着薬の薬効や無機P過剰摂取のスクリーニング,食事療法効果の評価などが可能となる.標準化蛋白異化率およびP除去量両者の評価では,高齢者において標準化蛋白異化率,たんぱく摂取量,P吸収量のいずれもが低値であった.また,若年層に無機P過剰摂取が多いことが示唆された.

[ Summary ] An excess of phosphate is a major risk factor for death in hemodialysis patients. Currently, predialytic phosphate concentration has been used as the only guide for understanding phosphate metabolism. We proposed that estimation of the amount of phosphate removal could be useful for evaluating phosphate metabolism. In addition, we established a simple method for the accurate estimation of phosphate removal. Because phosphate mass balance is maintained, almost the same amount of phosphate absorbed through the intestine is eliminated through hemodialysis. Therefore, estimating the amount of phosphate removal provides an estimate of the amount of phosphate absorption. Using the estimation of phosphate removal, we can evaluate the effect of phosphate binders, over-intake of inorganic phosphate, diet therapy, and other factors. Additionally, we evaluated protein and phosphate intake in hemodialysis patients. Low neutralized protein catabolic rate, low intake of protein, and low absorption of phosphate were observed in elderly patients. On the other hand, young patients were suggested to have over-intake of inorganic phosphate.
