【 要旨 】 本稿ではリハビリテーション栄養療法の基本的な考え方を提示し,症例を提示してリハビリテーション栄養の実践的な方法を示した.高齢透析患者は,保存期からある程度たんぱく質制限があり,透析が導入されると食思不振の状態にも陥る.また,透析導入による倦怠感で不活動となり,加齢の要因も加わってサルコペニア状態となる.リハビリテーション栄養では,国際生活機能分類(ICF)で評価を行ったうえで,障害者や高齢者の機能,活動,参加を最大限発揮できるような栄養管理を行うことが重要であるが,高齢透析患者にも臨床応用の適応となりうる.そのためには,関連職種との連携は必須であり,今後,この分野でのエビデンスの早急な構築が望まれる.

[ Summary ] This study presents the basic idea of rehabilitation nutrition : a combination of rehabilitation and nutritional care management. This concept in accordance with the International Classification of Functioning, Disability, and Health guidelines evaluates the nutritional status and maximizes functionality in the elderly and people with disability. Rehabilitation nutrition may improve physical and mental function, activities of daily living, and the quality of life. Rehabilitation nutrition is similar to sports nutrition and its key aims are to assess the following : the presence and cause of malnutrition, the presence and cause of sarcopenia, the presence and cause of dysphagia, the adequacy of nutritional care management with prediction of future nutritional status, and whether rehabilitation such as resistance training and endurance training can be conducted for functional improvements. However, elderly patients with chronic kidney disease (CKD) and dialysis are at higher risk of morbidity and mortality, and sarcopenia. In addition, sarcopenia is strongly associated with an increased mortality risk in patients with CKD. Therapeutic strategies including exercise training and essential amino acid and vitamin D supplementation may improve sarcopenia and physical inactivity in these patients and may improve the disease prognosis for these patients.
