| 【 要旨 】 | 介護保険制度は，高齢者の実態と社会情勢の変化に応じ，3年ごとの改正が行われてきた．2015年4月の改正では，「地域において効率的かつ質の高い医療提供体制を構築するとともに，地域包括ケアシステムを構築することを通じ，地域における医療及び介護の総合的な確保を促進する」という趣旨のもと，医療・介護・予防・住まい・生活支援の包括的なサービス提供を実現していく方針が示された． 透析医療従事者には，介護保険などの制度への理解を深め，他職種と連携して社会資源の有効活用を行い，透析患者を支援していくことが期待されている． |

| [ Summary ] | Long-term care insurance system has been revised every three years to cope with the actual situation of elderly people and the changing social conditions. The recent revision in 2015 calls for the "construction of the effective and high quality medical offer system in a community, and general promotion of the medical and care services in a community by building the integrated community care system." The policy aims at the construction of an integrated community care system to provide medical and care service, preventive medicine, residence, and life support. In order to support dialysis patients and cooperate with other professionals, medical workers should stay updated on long-term care insurance system and related issues as well as utilize social resources effectively. |
