【 要旨 】 わが国は世界に類を見ない高水準の医療制度を確立し，高齢化が進行し，2050年には20～64歳1.2人が，65歳以上の高齢者1人を支えるという厳しい社会が訪れることが予想されている．また，高齢認知症患者も増加する．このような傾向は透析患者においても同様であり，今後は，高齢透析患者が自立して，生活活動度を維持しながら透析医療を受けることができるようなシステム作りが重要である．腹膜透析（PD）などの在宅医療用具の開発・普及により透析患者の活動領域を拡げることを厚生労働省も推奨している．今後は安全なPD医療を推進するためにも，地域連携システムを推進することが必要となる．

[ Summary ] Japan has established a high level health care system, which is unique in the world. Our population is aging, and it is projected that we will have a society in which 1.2 people aged 20 to 64 will support one elderly citizen over 65 by 2050. In addition, the number of elderly patients with dementia is increasing. This trend is true also for dialysis patients. We must create a system in which elderly end stage kidney disease patients can receive dialysis treatment while maintaining active lives. The Ministry of Health, Labor and Welfare recommends expanding the activity levels of dialysis patients by developing and disseminating in home medical equipment. Therefore, peritoneal dialysis is a suitable treatment modality. In the future it will be necessary to create a system to promote regional cooperation between medical staffs for safe PD treatment.
