【 要旨 】 腹膜透析（PD）は高齢者の腎代替療法としてよい適応であることは周知の事実であり一定の良好な成績も報告されている．PD導入の際に，最初に越えなければならない重要な関門はペリトニアルアクセス（peritoneal access；PA）の留置術である．十分に機能するPAが存在してこそ良好なPDが維持できる．高齢者に使用するカテーテルや留置術に関しては，他の年齢層の患者のそれと基本的なコンセプトに差異はないが，高齢者に特有な，身体的・精神的な特徴や衰え，陥りやすい合併症に対しては相応の注意を十分に払う必要がある．

[ Summary ] Peritoneal dialysis is a suitable modality for renal replacement therapy for senile patients undergoing dialysis. It is commenced with the exclusive placement of a peritoneal access into an abdominal cavity. A fully functioning peritoneal access promises improved clinical course of peritoneal dialysis. Therefore, great attention should be given to creating a peritoneal access in senile patients with chronic renal insufficiency.
